Caregiving is one of the greatest expressions of love. It is also one of the greatest responsibilities a person can carry.

Every day, millions of family caregivers quietly place their own lives on hold to care for aging parents, spouses, grandparents, siblings, veterans, and loved ones living with dementia, Alzheimer's disease, Parkinson's disease, disabilities, and chronic illnesses. They prepare meals, coordinate medical appointments, manage medications, advocate within healthcare systems, balance careers, raise families, and provide emotional support, often while neglecting their own health and well being.

Most never imagined becoming a caregiver.

Yet every day they wake up and choose love.

The Caregiving Gap

Some families earn too much to qualify for traditional assistance, but not enough to comfortably afford private caregiving services.

They are often forced to choose between work, household expenses, their own health, and caring for someone they love.

The Anchored in Care Exchange was created for these families.

Through the Community Care Exchange, caregivers and families can access practical support, companionship, caregiver relief, resource navigation, and meaningful community connection. By bringing together volunteers, community partners, and caring individuals, we help families receive support when traditional assistance is unavailable and private care is out of reach.

Built From Lived Experience


The Anchored in Care Exchange was founded by caregiver and Certified Life and Relationship Coach Kisha Reynolds. As the primary caregiver for her mother living with vascular dementia, Kisha experienced firsthand the emotional, physical, financial, and social realities of caregiving.

Her experience inspired a community-based model designed to ensure that caregivers and families do not have to walk this journey alone.

Community Care, Built Around Families

The Anchored in Care Exchange is a nonprofit helping caregivers, older adults, and families navigate the caregiving journey through community, compassion, education, and shared responsibility.

At the center of our work is the Community Care Exchange Program, which connects families with trained volunteers, trusted resources, educational support, and community partners. Together, these connections help caregivers feel supported while helping older adults remain engaged, valued, and connected.

The Community Care Exchange Program is beginning its pilot in Philadelphia County, with a long-term vision to expand throughout surrounding counties, Pennsylvania, the Tri-State Area, and communities nationwide.
